LAB hours: 8:30am - 11:30am and 2:30pm - 4:30pm Monday through Friday. NO PT/INR (Coumadin checks) drawn on Friday.

Driving Directions:

Our clinic is located off the Woodburn exit of I-5.

At the exit proceed East on Hwy 214 to Evergreen Road (Arco &76). Turn right onto Evergreen Road.

Our clinic is located on the Southwest corner of Evergreen Road and Stacey Allison Way.

Parking is available in front of the clinic. The building has handicapped accessibility.
